Great tips from two great teachers/intensivists! A couple thoughts I had as I listened: (1) For the normotensive LV cardiogenic shock patient, I’d like to plug afterload reduction with something like nitroprusside (https://doi.org/10.1016/j.jacc.2008.02.083). More energetically favorable to reduce afterload than to try to augment contractility when you can. (2) Definitely a fan of vaso in the RV failure patient for protecting the systemic blood pressure.
